If you would like to request a copy of your medical records, they can be obtained through our ShareCare portal. To start the process, please select the applicable link below:

Patient Medical Records Request:  For patients and legal guardians with legal paperwork. You will be required to provide a valid email address and a government-issued ID.

Third Party Medical Records Requests: For healthcare providers, attorneys or if you are requesting the medical records for someone other than yourself and are not the legal guardian. Please use this link to upload your request along with the patient’s authorization.
